Course Information Course Title Course Number Units I plan to develop the course and submit all materials in a suitable format on or about Required Approval Submission of this application indicates a commitment on the part of the faulty member to teach, grade assignments and provide timely feedback to students for at least three years unless the instructor leaves the University. In the event that an instructor leaves the University or chooses not to teach the course after three years, the department head retains the right to continue using the course materials and to pursue another instructor to continue teaching the course. The department agrees to review the course prior to submission and certify its academic integrity as an acceptable credit offering. The department also agrees to nominate a qualified faculty member to teach the course until it is withdrawn from general use. The faculty members will be approved by the Office of Online and Distance Education. A periodic review will be undertaken to ascertain if a revision is needed. Note Contact Information
